In collaboration with all 17 Central East ADP providers, Seniors Care Network led the Developmental Evaluation of the Hybrid Model from July 2020 to Mar 2021. The intent of the evaluation was to ascertain which high-performing elements of the Hybrid Model should be permanently adopted post-pandemic, while identifying opportunities for quick course correction. All modes of delivery were evaluated using three main criteria: 1) operational barriers and facilitators, 2) quality of programs and services and 3) cost of delivery.
