2021 Seniors’ Care Excellence Awards

The Seniors’ Care Excellence Awards aim to recognize the outstanding contributions of individuals and organizations in the improvement of seniors’ care in the Central East Region.

To nominate an individual or organization, or to submit your abstract for consideration, please complete the appropriate form (below). Please review the included award criteria and submission instructions carefully.

All forms and any supporting documentation (if required) must be submitted together, no later than: Monday, May 31, 2021 by 11:59 p.m. 

The Awards will be presented virtually in the month of June.

Zahra Abdille Clinical Excellence Award

This award recognizes the outstanding contribution of a clinician/health professional in the delivery and improvement of clinical care to seniors in specialized geriatric services. This individual demonstrates: (a) exemplary leadership and teamwork qualities; (b) innovative and creative thinking; and (c) commitment to enhancing quality of life of seniors with complex health conditions. Nominations are invited for individuals working/practicing in area of the Central East Region.

Senior Advocacy Award

This award recognizes the outstanding contribution of an individual member of the community who has volunteered their expertise, time, and passion to improving the care and quality of life of seniors and care partners in the Central East Region.

Leadership Award

This award recognizes the outstanding contribution of a clinician/health professional or staff member working in or supporting specialized geriatric services. This individual demonstrates: (a) exemplary leadership qualities; (b) innovative and creative thinking; and (c) commitment to enhancing services for seniors with complex health conditions. Nominations are invited for individuals working/practicing in area of the Central East Region.

Teamwork Award

This award recognizes the outstanding contribution of a clinician/health professional or staff member working in or supporting specialized geriatric services. This individual demonstrates: (a) exemplary teamwork qualities; (b) innovative and creative thinking; and (c) commitment to enhancing services for seniors with complex health conditions. Nominations are invited for individuals working/practicing in area of the Central East Region.

Pandemic Champion Award

This award recognizes the outstanding contribution of a specialized geriatric services clinician/health professional or staff member during the COVID-19 Pandemic. This individual has demonstrated exemplary kindness, compassion, and a deep sense of commitment to helping others during the Pandemic. Their actions have made a meaningful impact on the lives of seniors and their families. Nominations are invited for individuals working/practicing in area of the Central East Region.

Community Partner Award

The Community Partner Award seeks to recognize the outstanding contribution of an organization/agency that has made considerable efforts to partner and collaborate with specialized geriatric service (SGS) providers in the Central East Region. This organization/agency demonstrates a commitment to improving the system of care for seniors and their care partners.

Poster Award

This award recognizes the highest rated poster submission, as determined by the Award Committee. Poster submissions are invited from individuals or teams working/practicing in area of the Central East Region. Poster content must be related to seniors (research, quality improvement project, or practice-based). This can include any poster you have submitted (or intended to submit) for a conference.

Senior Advocacy Award

Recipient: Randy Filinski
The Senior Advocacy Award seeks to recognize the outstanding contribution of an individual who has volunteered their expertise, time, and passion to improving the care and quality of life of seniors in the Central East LHIN.

This award recognizes the outstanding contribution of an individual clinician/health professional in the delivery and improvement of clinical care to seniors. This individual demonstrates: (a) exemplary leadership and teamwork qualities; (b) innovative and creative thinking; and (c) commitment to enhancing quality of life frail seniors.

 

Community Partner Award

Recipient: Alzheimer Society of Durham Region
The Community Partner Award seeks to recognize the outstanding contribution of an organization/agency that has made considerable efforts to collaborate with specialized geriatric service (SGS) providers in the Central East LHIN in order to improve the system of care for seniors and their care partners.

 

Conference Poster Competition Award

Recipients: Revital Shuster, Sabrina Gaon, Supriya Gade, and Ethel Kaiserman
Title: “Innovation in Collaborative Acute Care Community Relationships to Enhance the Well-being of Hospitalized Frail Older
Adults upon Discharge”
This award represents the highest rated abstract submission, as determined by popular vote.

This award represents the highest rated abstract submission, as determined by the 2017 Our Choice Award Committee, a committee comprised of older adults from across the Region.

Title: “CATCH – Care after the Care in the Hospital”.
This award represents the highest rated abstract as determined by the Our Choice Award Committee, which represents a diverse body of seniors and caregivers from the Central East Region. The award was presented to the individual whose abstract most clearly demonstrates a commitment to improving the care experience for older adults.
