Nurse Practioners Supporting Teams Averting Transfers (NPSTAT)

Program Lead: Jeffrey Gardner

NPSTAT Program LeadJeffrey Gardner is the Clinical Director of the NPSTAT Program and a Primary Health Care Nurse Practitioner.

Prior to this role, Jeff served as an NPSTAT Nurse Practitioner supporting the Northeast Cluster of the Central East LHIN. He’s worked in a variety of clinical and administrative roles in public and private sectors, bringing together his in-depth knowledge of the long-term care system and his background in critical care nursing — both acute and primary care.

Jeff has been an on-going part of NPSTAT’s development, ensuring program alignment with Central East Local Health Integration Network, Ministry of Health and Long-term Care, and Central East Community Care Access Centre priorities. Under his leadership, NPSTAT is focused on ensuring its services are delivered in a way that improves access to high-quality care, creates readiness for future health system changes, and makes the best use of the public’s investment in their health care.

BSO Program LeadKaren Lee-Boulton provides leadership to ensure the BSO Program and the health care professionals and people it is intended to support benefit from shared and standardized ways of providing care and support. With over twenty years of experience leading quality improvement initiatives for financial, telecommunication and healthcare organizations, Karen contributes her skills and talents to develop, deliver and improve ways to enrich quality of life for people with responsive behaviours and their care providers.

Program Lead: Stacey Hawkins (Seniors Care Network) and Sarah Gibbens (Regional GEM Spokesperson)

GEM Program Regional SpokespersonSarah Gibbens is currently the GEM Program Regional Spokesperson and serves as a GEM Nurse at Northumberland Hills Hospital in Cobourg. Sarah enjoys serving in this advanced practice nursing role, providing the perfect balance of clinical complexity, while also enabling her to share her passion for gerontology with colleagues, ultimately benefiting frail seniors receiving their care. Sarah is also involved in several hospital and community initiatives that focus on the provision of and improved access to best care for seniors. She brings to this role a degree in Gerontological Nursing and a Masters of Nursing from the University of Toronto.
